Literature Order Form

There are 10 separate pieces of literature available (see images and order form below).

MFMA members are entitled to 75 total pieces of literature at no charge (max of 25 copies of any one item) per membership year; additional copies available at the listed cost, plus shipping.

Pricing and Shipping:

• The first 25 items are FREE with FREE shipping.
• Up to 50 items are FREE (max of 25 copies of any one item). Additional copies of the same item over 25 are at cost. $15 for shipping in the US and $20 for shipping in Canada.
• Up to 75 items are FREE (max of 25 copies of any one item). Additional copies of the same item over 25 are at cost. $20 for shipping in the US and $25 for shipping in Canada.
• Contact the MFMA for pricing on quantities over 75 total items.

Non-members are limited to one copy of each at no charge.

Ordering: Enter total quantities requested in the spaces provided below. Free quantities will be deducted from the final total.
